An untrained runner who makes it that far is asking a poorly prepared organ to do its hardest work at its weakest moment.

He notes that for runners who do not train and do not approach the race with caution, the stress on the heart is greater than it would be for someone with adequate preparation. First-time runners and those who undertrain face disproportionate strain. Cardiac biomarkers are elevated in up to 50% of marathon runners after crossing the finish line. In untrained runners, these markers may indicate more serious damage.

Fuel Demands Your Body Cannot Meet

A trained runner learns to time carbohydrates, fluids, and supplements like energy gels or electrolyte tablets across the 26.2 miles. The body requires steady glycogen replenishment to avoid collapse, and knowing when to consume what comes from practice runs. Without this rehearsal, an untrained runner has no sense of personal intake needs or tolerance.

Attempting to fuel mid-race without prior testing often leads to gastrointestinal distress, nausea, or vomiting. The gut needs conditioning to process nutrition while under physical strain. Guessing at hydration and calorie timing during the actual marathon compounds the already elevated cardiac and muscular risks.

Your Muscles Will Break Down

Under-training is a documented risk factor for the renal failure that sometimes accompanies this condition. Heat and Conditions Will Expose You

Weather adds another variable. At the 2024 Boston Marathon, approximately 2,974 runners received medical treatment. Of those, 126 were transported to hospitals. Heat was the primary factor. Trained runners know their limits in hot conditions. They have practiced running in warmth, learned their sweat rates, and adjusted their pacing accordingly. An untrained runner has none of this information. They do not know how their body responds to heat under load because they have never tested it.

Course conditions, elevation changes, and crowd density all affect pace and exertion. A trained runner has practiced adapting to these factors. An untrained runner has to figure it out on race day, while already managing a body that is struggling to keep up with basic demands.

Joints and Bones Take a Beating

The impact forces of running are absorbed by joints, tendons, ligaments, and bones. Training allows these structures to strengthen gradually over time. A runner who has been putting in consistent mileage develops denser bones and tougher connective tissue. Someone who has skipped this process shows up with structures that have not been conditioned for the repeated stress of 26.2 miles.

Stress fractures, tendon inflammation, and joint damage are common in undertrained runners. These injuries can take months to heal. Some become chronic. The immediate goal of finishing the race gives way to a long recovery period that prevents any running at all.

Finishing Does Not Mean Succeeding

Some untrained runners do cross the finish line. The body is capable of remarkable things under duress. But finishing a marathon without training does not mean the body handled it well. Internal damage can occur without obvious external symptoms. Elevated cardiac markers, kidney stress, and muscle breakdown may be present even in someone who walks away under their own power.
